Arbor Village Receives CMS Five-Star Rating

Arbor Village, Sapulpa’s finest inpatient physical rehabilitation and senior living community, recently
achieved the coveted five‐star rating for nursing care from the federal Centers for Medicare and
Medicaid Services (CMS); a distinction that few nursing homes in the State of Oklahoma have achieved.

The CMS five‐star rating is an evaluation system that helps seniors, families, and caregivers compare
nursing facilities more effectively. It covers standard and complaint surveys that are conducted over
three years, with on‐site inspections. The rating of quality measures includes an evaluation of 16
different physical and clinical measures for nursing home residents. Each facility is provided a rating
between one and five stars based off its combined performance.

Ernie Johnson, the Executive Director for Arbor Village since June 2015, commented, “It has been such a pleasure watching this community go from a three‐star to a four‐star and now a five‐star. When we
recently lost a long‐term care resident, who had been a part of this community since 1979, all of our
staff remained at her side because that is what we do. We treat our residents like family and that is why
we continue to treat generation after generation of family. We have grandmas and grandpas, retired
physicians, and local politicians that choose to come here for skilled rehab or who simply just want to
make this place their home. We are embedded in this community and we love what we do here.”

Jami McCleery, staff Advanced Certified Medication Aide, stated, “We are family oriented and work
together as a team to provide the upmost care possible.” Jami recently was awarded the Key to Quality
Award, while attending the National Association of Healthcare Assistants Convention (NAHCA) in
Washington, DC.
